Hello Dr. Jon Mendelsohn back today to show you how a sub nasal lip lift is performed. First of all the sub nasal lip lift is a procedure that can restore natural harmony and balance to the upper lip and nasal region.

For many reasons the upper lip lengthens and often becomes thinner with age. While Botox and lip fillers are other adjunctive procedures for establishing a more balanced and youthful lip.

Here I’ll show you how thesub nasal lip lift as performed. It’s a relatively simple but I never really like to use that term, procedure.

It begins by properly marking the lip beneath the nose here in what we call the nasal sill. This is ultimately where the scar will hide. Commonly call the bullhorn incision and you’ll see why in a moment, we will measure the length of the philthrum, That is the portion of the upper lip from the nasal sill to the vermillion border or what we call the red white line of the lip where our lip naturally has this little roll. So typically we use a normal length in women to be 13. Well eleven to 13 millimeters and a little bit longer and men 13 to 15 millimeters. That being said there’s no absolute right number. It’s about balance and it’s about what we’re trying to accomplish for each person. Some people want more Shortened philthrum Some people want a more natural look and so forth. So we spend almost as much time measuring and marking as we do with the actual surgery which is true for other procedures as well. So with Wendy here we wanted to create a noticeable but more subtle lip lift and therefore her lip which we measured started at approximately 20 to 21 millimeters in length. And so we plan to remove about six to seven millimeters That’s about six millimeters.

So once this area is marked and everyone’s happy we will localize the region. That means we’ll simply numb the area with. The lidocaine that we’re injecting quickly will numb it. So you’ll feel a tiny pinch and a stick it’s a little bit sensitive to start with, but the epinephrine also in the solution is responsible for constricting the blood flow what we call vaso constriction so the tissue will blanch and you’ll see this before we get started. And the purpose of this is so that we won’t have much bleeding as you know as you probably know the lip is a really vascular region and it’s got a great blood supply if you’ve ever cut your lip, you know just how much it bleeds. So we want to reduce the bleeding as we do with all surgery. And so we allow the lidocaine with epinephrine to do its thing so that we don’t have to worry as much. So the next step then is once we are Vaso constricted and we’re ready to go we’ll make an incision sharply. We take this little 15 blade a little knife and well we’ll make an incision through the skin in both the nasal sill area and down to the line that we measured very carefully and marked Any little blood vessels that we see can be cauterized here until there isn’t any bleeding. Once the skin is excised and we have no bleeding Then a meticulous closure is performed. I use both deep sutures to close the dermis and these will eventually dissolve on their own. For the skin closure. I will use dissolving all or perhaps some Prolene or nylon these are permanent sutures to very meticulously close the skin.

These are all removed about the sixth or seventh day following surgery. But that’s really all there is to it is take some time for all of the suturing but it’s a wonderful procedure that many people don’t even know exist. It’s often performed in conjunction with other procedures as we’re doing here with Wendy. She’s also undergoing an upper blepharoplasty an eyelid procedure as well as an advanced facelift to rejuvenate the lower portions of her face. These are all performed under local anesthesia.

A sub-nasal lip lift removes a portion of the skin under the nose and gently lifts the skin to reshape the Cupid’s bow and create a lifted appearance.
